How the ophthalmoscope is used
The ophthalmoscope shines a beam of light through the pupil and lets the examiner look along the same path at the lit back of the eye. The lens wheel corrects for the refractive error of both patient and examiner, so the retina, the optic disc and its blood vessels come into sharp focus.
The filters and apertures adapt the view to the task: the red-free filter makes blood vessels and small haemorrhages stand out dark against the retina, the blue filter is used with fluorescein dye to show corneal scratches, the small aperture reduces glare through an undilated pupil, and the fixation star gives the patient a target while the macula is checked.
- Dim the room lights and set the lens wheel to 0.
- Ask the patient to look at a distant point, and approach from the side along the line of the pupil.
- Turn the lens wheel until the retinal vessels are sharp, then follow them to the optic disc.
- Change filters or apertures as needed, and examine the macula last.
