How blood pressure is measured
The cuff is wrapped round the upper arm and inflated with the bulb until it squeezes the brachial artery shut. As the pressure is let down slowly through the release valve, the examiner listens over the artery with a stethoscope. The first tapping sound (Korotkoff phase I) marks the systolic pressure, and the point where the sounds disappear marks the diastolic pressure.
An aneroid gauge measures pressure mechanically, with a sealed metal capsule that flexes and turns the needle, so it contains no mercury and is light enough to carry between wards and on home visits. With the gauge built into the bulb, one hand can inflate, deflate and read at the same time.
- Seat the patient with the arm supported at heart level and wrap the cuff snugly round the upper arm.
- Place the stethoscope over the brachial artery at the elbow.
- Inflate to about 30 mmHg above the point where the radial pulse can no longer be felt.
- Deflate at about 2 to 3 mmHg per second, noting the systolic and diastolic readings.
