How the electrosurgical unit works
Electrosurgery passes radio-frequency current, here at 330-430 kHz, through tissue. At this frequency the current does not stimulate nerves or muscle, but it heats the cells it passes through. A continuous waveform heats cells so fast that they vaporise, giving a clean cut; interrupted waveforms heat more slowly, drying and sealing small vessels, which is coagulation. Blend modes mix the two so that the surgeon can cut and control bleeding together.
In monopolar mode the current flows from the active electrode in the surgeon's hand, through the patient, to a return electrode pad on the skin. The return electrode safety system monitors that connection automatically, the main safeguard against burns at the pad site. In bipolar mode the current flows only between the two tips of bipolar forceps, which suits delicate work close to nerves and vessels.
- Place the return electrode pad on clean, dry skin over a well-perfused muscle and connect it; the unit checks the connection.
- Select the mode and set the power, or choose one of the pre-set programs.
- Activate CUT or COAG with the hand switch on the monopolar handpiece or with the yellow and blue foot pedals.
- Watch the front display for settings, alarms and error messages throughout the procedure.
